About the Post:
An exciting opportunity has come available for a full-time permanent assistance social care practitioner this will be a peripatetic position covering the locality teams across adult social care. Also, there is a short-term temporary position within the reviewing team which will give someone the chance to gain experience working as an assistant social care practitioner.
We are looking at recruiting an enthusiastic and committed individual(s) to join our established team of Social Workers and Assistant Social Care Practitioners. The role requires a confident individual who can assist in the completion of service user and carer assessments of care needs and undertake reviews. You will be required to be creative in coordinating care planning, commissioning, and provisioning services to meet desired outcomes in line with promotion of early intervention and strengths-based social work.
You will be supported to work autonomously and carry an individual caseload, but equally work as part of a team to meet service demands. You will need to have some experience of working in health and social care settings and demonstrate skills in work prioritisation, identifying risks, organising your own workload, and working in accordance with all statutory frameworks and relevant social care legislation, council policies, and procedures.
