An exciting opportunity has arisen to join our service as an Associate Practitioner. You will be working independently and accountably as part of the Nursing team and wider Multi-Disciplinary Team: offering physical health care assessments alongside psych-social support/education.



You will act as a role model to other colleagues within the Service. You will work collaboratively with key partner organisations, service users, clinicians and other practitioners with the collaborative community based services.



You will deliver high quality, effective operational service to people with a range of mental health needs in accordance with the principles and objectives of the Trust and its vision as identified in the service. The position requires the successful candidate to have an understanding of physical health assessments and phlebotomy however additional training will be provided.



Working as part of the Multi-Disciplinary Team to promote the physical and emotional well-being of service users who present with a range of mental health conditions.



A key aspect of this role is that of undertaking physical health assessments for service users under the care of the Specialist Community Mental Health Service and working with system partners to promote well-being.



The post holder will work under the direction and care planning of a registered clinician and will attend the weekly Multi-Disciplinary Team meeting; participating in discussions to support risk assessment and risk management.

To be responsible along with other members of the team for the implementation of all care aspects of individual care planning. Attend to the physical and psychological needs of the service users under the care of the service; undertaking physical health care assessments of service users and acting on findings. To take a lead role for physical health monitoring, on behalf of the service. Produce accurate and timely documentation / records on all clients in line with relevant codes and Trust Policy. Contribute to assessment and observation of service users and provide relevant and timely feedback. Contribute to risk assessment of the individual and of the potential hazards in the service user environment. Attend the weekly MDT meeting. Report and document incidents as required. Act as co-worker (Under the supervision of a registered clinician). Under the supervision of a registered clinician take part in assessing, planning and reviewing care. Provide support to service user to promote concordance with medication and treatment regimes in line with Trust Policy. Transport and store medical equipment in line with Trust Policy. Provide practical support, guidance and education for service users. Record all service user related activity information into Paris. * Deliver smoking cessation training.
