Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. Our team at Farleigh has grown and evolved to meet the changing needs of the community we serve and we are looking for highly motivated Associate Practitioner to join our Therapy team.
As an Associate Practitioner, you will work within a multi-disciplinary team (MDT) to support the delivery of high-quality palliative and end-of-life care. You will assist in holistic assessments, provide emotional and practical support, and contribute to care planning and rehabilitation services under the supervision of registered professionals.
You will be empathetic, with excellent communication skills and a passion for service improvement. You will preferably have experience of working with people at the end of their lives. You'll be confident in engaging in sensitive conversations and supporting patients with complex needs. You'll also help facilitate group sessions, promote non-pharmacological symptom management, and support patients in accessing appropriate health and social care services.
You will be a proactive problem solver with a "can do" attitude, able to work both independently and under the supervision of the registered therapists. You'll contribute to service development, quality improvement initiatives, and help maintain strong relationships with external partners and stakeholders.
You will have completed the Assistant Practitioner Foundation Degree course with either an Occupational Therapy or Physiotherapy module.
This is a fixed term contract and will be reviewed at the end of the term with regards to the needs of the service.
