To work as part of urgent community response working in patient's homes across West Suffolk to deliver high quality nursing care.
You will assess, plan and deliver planned/unplanned healthcare for patients often with long term conditions and interventions will include: end of life care, palliative care, catheter care and wound care. You will want to enable patients to achieve quality of life, dignity and comfort.
You will be passionate about delivering holistic care to patients and their families.
You will be supervised by a nurse within the team and have access to training and learning opportunities across the Trust. If you are newly qualified, you will also be supported with preceptorship.
You will work as part of an MDT who support urgent community response and at times will complete joint visits with ACPs and therapists. As a team we intercept some ambulance referrals to provide an urgent response. This is an exciting time to work in the community with developments to support care being closer to home.
Triage, assess, plan, implement and evaluate all aspects of patient care, mainly unplanned to include palliative and end of life care, catheter care and wound care.
You will need to be able to drive to patient's homes and have access to your own car.
Following holistic assessment, devise an individualised, evidence-based plan of care for each patient, modifying it as required.
Assess patients' holistic needs, communicating complex and sensitive information to patients and carers.
Have a good understanding of palliative and end of life care, including symptom control and independent
management of syringe pumps and end of life medication.
Use tactful and persuasive skills to gain the patient's co-operation in their care management plan.
Liaise with GPs, Social Services, inpatient teams, hospice, other health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated.
Participate in multi-disciplinary/multi-agency meetings as appropriate.
To be competent to deliver IV therapy.

STAFF CAR PARKING
Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 - 18:30.
For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital. Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.
This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete "on call" duties within their role.
