Are you a support worker with experience looking for a new challenge within a community based mental health team? We are looking for people with the right values, skills and/or potential to join us. We are an exciting dynamic team that looks to improve and provide mental health services for the young people we support who aged between 18-25 years old who are in a mental health crisis.
Our unique service supports a variety of young people that have deteriorated into a mental health crisis that puts them at risk of being admitted to a mental health hospital. As part of the Crisis Home Treatment Team you will support young people in their homes to try and prevent admission. If hospital admission cannot be avoided, together as a team we support young people returning to the community following admission in their homes with their family, friends or carers.

Main duties of the job
You will be an inspirational individual - a warm and compassionate person - someone who is able to take direction and make safe and confident decisions around patient care in partnership with a range of other health care staff, acting as a vigorous advocate for our patients and their families. Having their voice heard is your main objective.
Within your role you should deliver evidence based interventions to positively effect change that safeguards and promotes the welfare of children, young people and their families/carers.
We expect our staff to lead from their heart and have a passion for wanting to make a difference to people's lives.
The post holder will be a required, to provide intensive support/intervention /advice and identify existing community resources to minimise/avoid escalation towards crisis by means of specialist assessments and evidence based interventions. A significant part of the role is to work collaboratively with the other agencies involved to avoid inpatient hospital admission.
You will have access to a range of specialists and professionals - ours is a truly multi-disciplinary approach to health care.
