We are looking for a compassionate, enthusiastic and caring Band 4 Assistant Practitioner, who is reliable and flexible to join our Intensive Dementia Service Team based in Yeovil



If you have good interpersonal and organisational skills, enjoy a challenge and have a passion to deliver a high standard of patient care, then this is the opportunity for you!

Main duties, tasks & skills required

You will provide support and assistance to Registered Staff and more junior colleagues as an important member team and under supervision perform a broad range of activities and assigned tasks as required by our clients individualised care plans.

In addition to the details in the supplied Person Specification/ it's important to note that any applicant would need to have significant experience and strong skills related to the direct care given to people with Dementia.



It's also essential to this community-based role that any applicant will need current driving licence and access to a car for work purposes (with appropriate insurance).
