We are looking for a compassionate, enthusiastic and caring Band 4 Assistant Practitioner, who is reliable and flexible to join our Intensive Dementia Service Team based in Yeovil



If you have good interpersonal and organisational skills, enjoy a challenge and have a passion to deliver a high standard of patient care, then this is the opportunity for you!


Main duties, tasks & skills required

You will provide support and assistance to Registered Staff and more junior colleagues as an important member team and under supervision perform a broad range of activities and assigned tasks as required by our clients individualised care plans.

It's also essential to this community-based role that any applicant will need current driving licence and access to a car for work purposes (with appropriate insurance).
