We, the Bury South Adult CMHT, currently have a substantive Band 5 Community Mental Health Nurse ( 34.875 hours) post available and are looking for a nurse to join our trust and bring fresh ideas and enthusiasm for developing the service.
Our team work together to support each other as well as giving you the autonomy to work with individuals, develop therapeutic relationships and support individuals to live as rich and meaningful a life as possible. We are a recovery focused, multi-disciplinary team providing an effective, supportive, and professional service for our Service Users and Carers.
We are looking for people to join us in building what the future service of looks like. You will be supported to develop a strong understanding of your role as a Care Coordinator/Assessor and join our dedicated multidisciplinary team working closely with a range of partners to provide appropriate community interventions for those individuals referred to our service.
Our aim is to continually improve service delivery, service user / carer experience, and work towards positive outcomes.
You'll be working alongside a multi-disciplinary team. You'll regularly be working with external agencies as part of a service users care, so being able to work well in a team and having excellent communication skills is vital. You can expect a thorough induction and training / preceptorship period, and opportunities for ongoing development.
You will enjoy plenty of opportunities for joint working, training, and a sound supervision structure.
You can expect a manageable case load and a supportive management team to ensure you're able to work independently but support is never far away if needed.
The ideal candidate should have experience in working in a mental health environment and ideally experience in working in community teams.
Working in a busy and dynamic Adult Community Mental Health Team means every day will be different. You will be involved in:
Managing a caseload via face to face and telephone contacts for those under The Care Programme Approach
Undertaking initial urgent/routine assessments on behalf of the service
Attending multi-disciplinary team meetings.
Attending outpatient clinic appointments
Undertaking face to face visits in the community setting including home visits
Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as Social Care, Crisis Resolution and Home Treatment Team and third sector organisations)
Medication administration
We also works with needs types and have many evidence based work books to enhance the Service user experience.
Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm)
