Are you a compassionate Registered Nurse (RN) seeking a new opportunity? We have a Community Staff Nurse position available within the Glastonbury team based at West Mendip hospital, but the team cross-covers the whole West Mendip area.
Our service covers the urban and rural areas of West Mendip. These positions are dayshifts and are usually 8am-4pm, 9am-5pm or 10am-6pm. You'll also work twilight shifts (2pm-10pm) and weekends which are shared between teams and these include additional unsocial hours enhanced payments, there are no night shifts. We offer flexible working solutions where possible to ensure a work-life balance.
You will need to be a car driver with your own vehicle to visit patients. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

Main duties, tasks & skills required

As a community RN, you'll provide care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines.
You'll also work with the Hospital @ Home and rapid response team, focused on preventing acute admission. You'll develop a broad range of nursing skills, including IV medication administration, palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management.
You'll need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team Leaders. Comprehensive training and guidance will be provided by our dedicated Clinical Lead nurse.

Community Nurses work as part of a team providing high quality and compassionate care to individuals. The Community Nurse is an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team. They will support the multidisciplinary team in the delivery of high-quality care to enable individuals to achieve their person-centred goals.
The Community Nurse will take responsibility of the patient's care pathway and non -clinical duties in the absence of the Team Leader. They will have the understanding to address complex problems in their day-to-day work, reporting concerns to a Team Leader.
The Community Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required. Community Nurses will operate within agreed ways of working, following the relevant standards, policies and protocols used in their workplace including the standards in the NMC Code, Professional standards of practice and behaviour for nurses and midwives.
