Job Purpose To deliver a high standard of specialist nursing care to babies, children, and young people (CYP) with complex health needs within a special school setting, their homes, and other community environments. Working as a vital member of the Special School and Complex Needs Nursing Team, the post-holder will assess, plan, implement, and evaluate care, managing a defined caseload under the direction and support of a Band 6 Nurse. The role requires close collaboration with families, carers, education staff, and the Multi-Disciplinary Team (MDT) to ensure integrated, holistic care that promotes the best possible health outcomes and quality of life.



2. Key Responsibilities Clinical Practice:

To assess, plan, implement, and evaluate programmes of care for CYP with complex needs, including those requiring enteral feeding, management of respiratory conditions, epilepsy, and other long-term conditions. To provide skilled, evidence-based nursing care, including clinical procedures and interventions, according to patient care plans and trust policies. To support the Band 6 in managing a defined caseload, ensuring care is patient-centred, safe, and effective.



To recognise and respond appropriately to changes in a childs condition, escalating concerns promptly to the Band 6 nurse or senior lead. To administer medication according to policy and patient-specific protocols. Communication and Relationship Skills: To establish and maintain supportive, empathetic, and professional relationships with children, young people, and their families/carers, often in stressful and emotional situations. To work collaboratively as part of the MDT, communicating effectively with colleagues in education, social care, therapy teams, and other healthcare professionals.



To provide clear, understandable health information and education to families, carers, and school staff, empowering them to manage the childs health needs confidently. To maintain accurate, timely, and legible electronic patient records in line with professional and trust standards. Leadership, Management and Team Working: To work independently within the scope of practice, while recognising the limits of their own competence and seeking support and direction from the Band 6 nurse as required. To contribute to a positive, supportive team environment.



To supervise and support junior staff, students, and volunteers as appropriate. To participate in team meetings and contribute to service development initiatives. Professional Responsibilities: To adhere to the NMC Code of Conduct, maintaining professional registration and responsibility for own continued professional development (CPD). To participate in clinical supervision, appraisal, and mandatory training.



To have a good understanding and working knowledge of safeguarding procedures for children and young people, acting promptly on any concerns in line with trust policy. To utilise IT systems effectively for patient records, communication, and data management. 3. Working Conditions The role is community-based, requiring travel between special schools, family homes, and clinic settings.



A valid UK driving licence and access to a vehicle for work purposes is essential (unless disability precludes this). The post-holder will be exposed to a range of clinical conditions and emotional situations. Agile working is supported, with a requirement to work flexibly to meet the needs of the service.
