MAIN RESPONSIBILITIES:

Job Summary:

The post holder in collaboration with the Matron and the Multi-disciplinary team is responsible for the delivery of safe, effective care leading to an excellent experience for patients and carers by.



Being responsible for ensuring high quality, evidence-based patient centred care is delivered and providing assurance of this whilst delivering the Trusts core objectives and strategies. Being responsible for setting the standards with the nursing team, HCA, Technicians and ensuring that the environment allows the workforce to feel supported to deliver safe and effective care. Ensure good inter-personal communication is achieved and build positive team working. The post holder will be the role model for the nursing staff providing clinical leadership and promoting the development and maintenance of quality standards. Manage day to day activities in Outpatient services. Able to work on the clinical area to support team and ensure to provide efficient and safe service in Outpatient setting.

MAIN RESPONSIBILITIES:

1. To lead the department as an expert clinical practitioner, ensuring that staff have appropriate skills necessary to care for the designated patient group.



2. To undertake specified nursing procedures and treatments incorporating, assessing, counselling, planning implementation, initiating, and evaluating care of patients



3. Provide supervision to staff and constructive feedback on Clinical competence.



4. To manage the nursing staff, HCA, Ophthalmic technician to provide high quality care to agreed standards and objectives.



5. Maintain up-to-date records and ensure that records are stored and filed in an orderly and safe manner. Ensure that confidentiality is respected, and that self and staff meet the Trust & NMC standards for "Record Keeping".



6. Ensure that ordering, storage administration and documentation of drugs are in accordance with Trust policy for the prescribing, administration, and control of drugs. Have accountability for the correct administration and custody of medicines according to the trust policy to include the safe administration of medicines, controlled, intravenous and cytotoxic drugs.



7. To undertake staff appraisals and to ensure that there is an ongoing appraisal programme and staff are supported to achieve their competencies as appropriate.



8. To ensure service needs are met by ensuring staff development is continued in training for biometry, Intravitreal treatment, IV cannulation, Minor Ops, Telephone Pre op assessment and other diagnostic training.



9. To maintain own development in Ophthalmic skills required for service development of current and future planning.



10. Support the Matron to lead, support and advise staff on the planning process to meet Outpatient targets.



11. Practice autonomously with a high level of accountability at an advanced clinical level in the direct delivery of high-quality care, making critical clinical decisions based on advanced clinical skills assessment.



12. Support your team to work in partnership with individuals, families, and carers, using a range of assessment methods as appropriate (e.g., holistic assessment, identifying risk factors, requesting, undertaking and/or interpreting diagnostic tests and conducting health needs assessments).



13. Challenge practice so that the best and most appropriate treatment is given to the patient, maintaining, and monitoring high standards of care, recognising, and responding proactively to patients' changing clinical needs.



14. Work autonomously, managing a group of patients to deliver high quality skilled, evidence based, and patient centred care supported by departmental Standard Operating Procedures, policies, and guidelines.



15. Understand the workload and targets that has been set for clinical area and demonstrate commitment to their delivery.



16. Practice as a professional nurse demonstrating leadership, clinical expertise, and competence in the field of Ophthalmic nursing.



17. Communicate and liaise effectively with internal departments and external agencies to arrange and confirm on going pathways of care, provide written communication and referral documentation as required.



18. Supervise and support junior members of staff in the delivery of patient care.



19. Be conversant with trust policies, procedure and systems for safeguarding including the assessment of Mental Capacity, the process for consideration requests for Deprivation of Liberty Safeguards (DOLS) and the application of the principles of Prevent (counter terrorism awareness).



20. Report and raise concerns about Safeguarding, accessing advice and support at the point of need.
