Barnet South Core Team are a friendly and supportive team of Community Therapists and Therapy assistants working with service users who have complex and substantial needs in the Edgware, Middlesex area.
Responsibilities
Main Responsibilities:
You will hold a case load across Barnet Community Core Hub to provide assessment, plan and implement interventions, evaluate treatment and ensure safe discharge.
You will work closely with the Occupational Therapist and Therapy lead.
You will work with service users who have complex and substantial needs, often working with people with dual diagnosis and co-existing conditions including physical health needs.
You will work closely with multi-disciplinary teams and relevant outside agencies.
To be successful in applying for this role you will need:
HCPC registration
Degree in Occupational Therapy
Understand MOHO ax tools
Demonstratable experience in completing ADL reports
