Key Responsibilities:

• Conduct assessments and develop tailored intervention plans for elderly patients with mental health needs.
• Collaborate within a multidisciplinary team to deliver comprehensive care and support.
• Implement therapeutic activities to improve patients' well-being and independence.
• Educate and advise patients, families, and carers to promote positive health outcomes.
• Ensure accurate and timely documentation to maintain high standards of care.

Preferred Experience and Requirements:

• Experience in mental health, particularly with older adults, is advantageous.
• Previous Band 6 experience is highly desirable.
• HCPC registration is essential.
• A valid driving licence is preferable to support client outreach.
