Hourly Rate: 23.82



Monday - Friday



08:00-16:00, 37.5 Hours



Please note this role is hybrid with some requirement to come into the office for meetings, travel to patient homes for clinical visits and the option for some working from home to complete clinical admin. Staff can choose to come into the office more regularly if they prefer.



Job Responsibilities:



To communicate effectively with all disciplines involved in a patients care, both those from within the organisation and those outside to ensure effective multidisciplinary working. This will include discussion of patient care, patient progress and involvement in discharge planning.

You will work as part of a multi-professional neuro rehab teams, in close collaboration with other relevant services/agencies e.g. acute rehabilitation units, community rehab teams, inpatient wards, rapid response etc.

You will manage a caseload of clients within the community or ward setting and oversee caseload for rehabilitation assistants.

You will undertake assessment and treatment of clients within a community/ward setting, who have complex / and or chronic presentation requiring specialist skills and determine profession specific and multidisciplinary rehabilitation interventions.

You will carry out profession specific and general multidisciplinary rehabilitation procedures.

To be successful in applying for this role you will need:



HCPC registration B.S.C Hons or diploma equivalent in Occupational Therapy You will be required to drive to patient homes so are required to have access to a car for the duration of your shift and have your car insurance set up for business use. Familiarity and confidence with using SystmOne patient systems is mandatory. Experience in working with patients who have neurological conditions is desirable.
