Paediatric Diabetes Specialist Nurse Pay Band: Band 6 Hours: 37.5 hours - part time applicants welcome to apply as there is some flexibility to accommodate a job share Contract: Permanent Location / Department: Worcester Royal Hospital, Paediatric Diabetes, Women and Childrens Directorate Accountable to: Paediatric Sub-Speciality Matron Reports to: Lead PDSN Key Working Relationships: Liaises with all appropriate members of the multidisciplinary paediatric diabetes team within the community, hospital, social, nursing, educational and voluntary sectors. Risk Assessment: The post involves manual handling activities and exposure to body fluids and chemicals. Will involve frequent VDU work. Individuals may be exposed to violence and aggression.
The role includes lone working. You will need to be able to drive and have valid insurance cover, risk assess locality and parking for home visits and be able to risk assess the home for hazards and potential risks including animals. Job Purpose: To work as a clinical nurse specialist in paediatric diabetes specialist care across the accepted domains of the role as a clinician, educator and researcher as part of the Countywide Paediatric Diabetes Team. To support the multidisciplinary team in line with the Best Practice Tariff, National Service Frameworks, government and Trust Initiatives for Paediatric Diabetes Care by being a key contributor to the on-going development of the service.
To work as a member of the paediatric specialist diabetes care team, to ensure a cohesive, holistic family centred, and research-based approach to the management of diabetes in children and young people. To participate in teaching programmes to parents/carers, children and young people and school/nursery staff. The role will be innovative and encompass all aspects of the patient care pathway, acting as a nurse advisor to support children and young people with diabetes, their families and other carers within a variety of settings. Key Duties: To autonomously manage a clinical caseload of young people with diabetes and their families.
Providing child focussed, holistic family centred care in various settings, including Acute Trust sites, home or community and educational establishments. To instigate and adjust treatment within set guidelines. To be a resource for all parties involved in a childs care including primary and secondary care staff, those in education and social care. To undertake nurse led clinics and baseline assessments, formulating clinical management plans.
To undertake specialist investigations, interpret results and initiate appropriate action. To participate in multi-disciplinary clinics To provide emergency support to families at times of crisis such as illness management and to participate in an on-call service when required Be responsible for promoting and safeguarding the welfare of young people in your care. To undertake management and family support of complex treatment options such as insulin pump therapy To advise clinicians on the adjustments of diabetes medication which need to occur, to ensure safe diabetes patient care. Act as a role model for junior members of staff and non-specialists.
To provide specialist teaching, education, support and advice for children and their families to allow them to become empowered to self-manage their condition, in a timely and professional manner. Work closely with all members of the multi-disciplinary team in both acute and primary care to promote the development of a seamless Paediatric diabetes service. Ensure that safe and effective nursing care is delivered, in accordance with Trust policies and best practice guidelines, also Nursing & Midwifery Council (NMC) Codes of Practice and guidelines. To provide support for a smooth transition to adult services To be prepared to participate in the annual Childrens Educational Activity Weekend and other organised extracurricular events.
Actively contribute to the health and wellbeing of patients and families by promoting health at all contacts. Ensure accurate, comprehensive contemporaneous and clear documentation of patients care. Also ensure that own and others written reports are in accordance with Trust policy and NMC guidelines Be familiar with the policy for safe administration of prescribed therapies, including Medicines Management working within Paediatric Diabetes guidelines. To adjust Insulin doses for young people in your care in line with the agreed guidelines for Worcestershire Acute Hospitals NHS Trust.
Assess those recently diagnosed as part of the continuing care for families and carers, where specialist intervention in required. Acts on the assessment to plan and implement as appropriate care strategy with support of the senior team members. To take a supporting role within the Paediatric Diabetes Team to meet the criteria for the best practice tariff and to contribute to service development, change management and policies and guidelines. Education Organise and run diabetes care study days and mandatory training for all disciplines of staff.
Take part in developing educational resources/programmes for key groups such as young people, parents, teaching staff and health care staff. Present on related courses, study days and training programmes to disseminate evidence-based diabetes are knowledge and practice. Provide education and training for patients, relatives and carers. Support undergraduate paediatric student nurse placements as directed by the senior team members.
Research and Audit Develop, implement, audit to measure and evaluate standards of practice relating to specialist diabetes care as part of the team. Ensure the dissemination of best practice. Adhere to the Trusts complaint procedure with regard to written and verbal complaints. Using appropriate referral mechanisms in order to resolve the situation and/or in conjunction with the PALS officer.
Individual staff have a role in suggesting and implementing approved improvements to services and exercise professional responsibility for both themselves and their peers within an open just and fair culture. Ensure clinical effectiveness by critically appraising current research and applying to practice. To co-operate and support clinical practitioners wishing to undertake research in diabetes care and take an active role in developing and participation in further research. Continuing Professional Development Maintain and develop specialist clinical knowledge, skills and competence through networking with other specialists and professional organisations.
Maintain a programme of professional development by attending in-house and external training, education, conferences and seminars in line with PDR/KSF review. As part of a joint commitment to lifelong learning, will contribute to the assessment of personal learning needs and opportunities to inform and develop personal development plans. Undertakes personal clinical supervision and supports others through clinical supervision to improve practice. Management Provides expert leadership and change management skills in developing the diabetes service.
Manage and lead your own caseload safely and effectively, prioritising and ensuring appropriate access to the service. Function effectively as a clinical nurse specialist in line with the recommendations of the NMC. Uses clinical supervision to maintain and improve practice Acts as a role model to uphold professional standards and high-quality diabetes care. General Duties: Maintains a programme of personal and professional development by attending in-service training and utilising relevant educational opportunities.
e.g., courses, conferences, seminars. Maintain effective registrations with the NMC, maintain personal, professional portfolio. Take personal responsibility for continuous professional development in line with PDR review and NMC revalidations process. Adhere to the policies, procedures and guidelines of Worcestershire Acute Hospitals NHS Trust relating to employment and professional practise.
