Main Duties of the Role



The successful candidate will have a one to two years' post-graduation experience and will work within their professional boundaries as part of the multi-disciplinary team at Heaton Moor Medical Group.

They will play a key role in ensuring seamless integration between our practices, community pharmacy, and hospital services to optimise skill mix, improve patient outcomes, enhance access to care, and support workload management.

This is a pivotal role focused on improving the quality of care and operational efficiency. The position requires a pharmacist who is motivated, passionate, and committed to delivering high-quality services within a general practice setting.

Job Responsibilities


Key Tasks

Manage and reconcile medicines for patients discharged from hospital, intermediate care, and care homes. Communicate medication changes to community pharmacists to ensure continuity of care. Work collaboratively within the multi-disciplinary team to lead medicines management across the practice. Take lead responsibility for the prescribing aspects of NHS contracts.

Responsibilities Underpinning the Role

Undertake clinical medication reviews with patients and provide prescribing and monitoring recommendations to nurses and/or GPs. Establish and manage systems to ensure continuity of medicines supply for high-risk patient groups (e.g. those with medicine compliance aids or living in care homes). Develop and maintain effective working relationships with community pharmacists and other local healthcare providers. Plan and organise personal workload, including audits, project work, and training sessions for the practice team. Maintain accurate, detailed, and up-to-date records of all audit, clinical, and technical work undertaken within the practice. Maintain professional registration as a pharmacist and adhere to relevant professional codes of conduct. Keep professional knowledge and skills current by engaging in continuing professional development, staying informed about clinical evidence, policy updates, and service developments, and participating in appraisals and personal development planning. Work collaboratively with the general practice team to ensure compliance with CQC standards related to medicines management. Ensure that all practice medicines management policies are regularly reviewed and updated. Where applicable, provide pharmaceutical advice in the development and implementation of new services with medicinal components (e.g. treatment pathways and patient information materials). Collaborate with the Health and Wellbeing coordinator to support public health initiatives and provide expert input into public health campaigns. Deliver education and training sessions for the primary healthcare team on therapeutics and medicines optimisation. Implement changes to medicines following MHRA alerts, product withdrawals, and updates from local or national guidance.

Qualifications

Essential



Undergraduate degree in pharmacy. Registration with the General Pharmaceutical Council (GPhC). Successful completion of one-year pre-registration training.

Desirable



CPPE Pharmacist in General Practicequalification pathway. Experienced and active independent prescriber.

Experience

Essential



One to two years' post-registration experience as a pharmacist, evidenced within a professional portfolio. Experience in recognising and managing common acute and long-term conditions encountered in general practice. Experienced independent prescriber, confident working autonomously within scope of practice. Demonstrates ability to prioritise effectively, recognise deviations from normal clinical patterns, and escalate to senior colleagues or GPs when appropriate. Adheres to legal, ethical, professional, and organisational policies, procedures, and codes of conduct. Involves patients in decisions regarding prescribed medicines and supports adherence in line with NICE guidelines.

Leadership

Demonstrates understanding of the pharmacist's role in clinical governance and applies this appropriately within the workplace. Contributes to the development and communication of the practice vision; engages with Patient Participation Groups (PPGs) to support role and service development. Demonstrates the ability to improve quality within existing service constraints. Reviews progress annually and develops clear action plans to achieve agreed objectives. Demonstrates self-motivation and commitment to achieving personal and professional goals.
