Job Introduction
We have the opportunity for a committed and friendly Nurse to join Chippenham Community team as one of our Community Nurse Specialist. We are looking for an inspirational Nurse to deliver a high standard of quality care to our patients and support the team with excellent leadership skills.
Our community teams deliver nursing care in patients' homes and residential homes as part of a Multi-Disciplinary Team, linking closely with the GP Practices and other community services in the local areas. We work with the aim of preventing unnecessary admissions to hospital and in assisting people to manage their health conditions to prevent hospital admission and promote independent living and health management.
Applicants need to have excellent clinical skills, well developed communication skills, be experienced in leadership and management and be able to work under pressure. The role is demanding but rewarding, and we are looking for an enthusiastic, motivated and hardworking individual who can work flexibly and efficiently. We are a busy, friendly team, who would welcome the opportunity to work with a new member of staff willing to contribute to the on-going development of the services we provide.
The team is based in Chippenham Hospital. The Team operates a shift patterns between 07.00 - 22.00. Nursing staff will be expected to work weekends and will be expected to cover early and late shifts., Evidence of up-to-date clinical professional development is essential and experience in front line NHS community patient care is preferable.

The Ideal Candidate
Our ideal candidate will be / will have:
Registered Nurse (RN) with current live PIN on NMC register
Teaching and Assessing NMC approved qualification
Evidence of relevant post registration specialist professional development
3 years' experience as a registered nurse
Experience in a variety of settings including hospital and community
Experience of managing a range of long-term conditions
Experience of working collaboratively with Social Services
Evidence of innovative clinical practice development
