Band 7 Childrens Community NurseService: Red Community Paediatric Nursing ServiceReports to: Clinical Nurse Lead / Community Services ManagerReference: B0654-25-0008 1. Job Purpose To play a key role in the delivery and development of the Community Childrens Nursing Service. The post-holder will be an expert clinical practitioner, responsible for the assessment, development, implementation, and evaluation of specialised nursing care for children and young people with acute, complex, palliative, and long-term health needs within the community setting. The overarching aim is to provide high-quality, family-centred care closer to home, preventing unnecessary hospital admissions and facilitating early discharge.



2. Key Responsibilities Clinical Practice To manage a defined, complex caseload autonomously, acting as a specialist resource for children, young people, their families, and other healthcare professionals. To undertake comprehensive nursing assessments of children and young people, formulating, implementing, and evaluating evidence-based care plans in partnership with the family. To deliver a wide range of advanced clinical nursing procedures in the community, including but not limited to: intravenous therapy, central line care, venepuncture, wound care, and medication administration.



To provide specialist nursing care for children with conditions such as oncology, long-term health conditions, and those requiring palliative and end-of-life care. To work proactively to prevent hospital admission and support the safe and timely discharge of children from hospital to home. Leadership and Management To provide clinical leadership, professional guidance, and supervision to junior staff, students, and support workers within the team. To deputise for the Clinical Nurse Lead as required.



To contribute to the development and improvement of the service through participation in audit, clinical governance, and the implementation of new policies and procedures. To effectively manage and coordinate the teams workload in the absence of the line manager, ensuring safe and effective service delivery. Communication and Partnership To establish and maintain effective communication and collaborative working relationships with a wide range of multi-agency partners, including GPs, hospital teams, social care, and educational settings. To empower and educate children, young people, and their families, enabling them to develop the skills and confidence to manage their health needs independently where possible.



To act as a clear and sensitive communicator, ensuring complex and potentially distressing information is conveyed with empathy and understanding. Professional Development To maintain and develop own clinical expertise and professional knowledge in accordance with NMC revalidation requirements. To participate in clinical supervision, appraisal, and personal development planning. To identify training needs and contribute to the education and development of the wider nursing team.



3. General Responsibilities To maintain accurate, contemporaneous, and comprehensive patient records in line with NMC and Trust guidelines. To use resources efficiently and responsibly. To ensure a safe working environment by adhering to Trust policies, including infection control, moving and handling, and health and safety.



To be a car driver with a valid UK license and have access to a vehicle for work purposes.
