Are you a dynamic, innovative, and enthusiastic Advanced Clinical Practitioner with a passion for supporting individuals with memory difficulties? Do you want to be part of a pioneering initiative that is reshaping Dementia and Frailty services across Birmingham?
We are excited to offer a unique opportunity to join a newly funded project under the Birmingham Fairer Future Fund , aimed at tackling health inequalities through culturally sensitive and collaborative care pathways. This role will be central to the co-design and delivery of new assessment models, working closely with system partners and community stakeholders to improve awareness and access--especially for under-represented groups.

Essential Criteria:
Registered healthcare professional with ACP qualification.
Experience in dementia care or memory services.
Strong commitment to reducing health inequalities.
Excellent communication and stakeholder engagement skills.
Ability to work autonomously and collaboratively in a multi-agency environment.
