Job Description

We are looking for motivated and adaptable pharmacists to join our Bank team, supporting the delivery of high-quality, specialist pharmacy services across a range of settings including clinical areas and dispensaries.

As a Bank Pharmacist, you will provide expert pharmaceutical advice to healthcare professionals and patients, promote safe, evidence-based and cost-effective use of medicines in line with the HDUHB Formulary, and ensure patient-centred care.



Deliver high-quality, patient-centered pharmaceutical care across a range of clinical and operational settings.



Provide expert advice on the safe, effective, and evidence-based use of medicines to prescribers, healthcare professionals, and patients.



Support dispensary services, including clinical screening, accuracy checking, and ensuring compliance with legal and professional standards.



Promote cost-effective prescribing in line with the Hywel Dda University Health Board (HDUHB) Formulary and local guidelines.



Collaborate with multidisciplinary teams to ensure seamless care and optimise patient outcomes.



Contribute to medicines reconciliation, discharge planning, and clinical reviews.



Support the education and development of pharmacy colleagues, including technicians, students, and trainee pharmacists.



Maintain accurate records and documentation in line with professional and organisational standards.



Demonstrate flexibility and adaptability to meet service needs across different sites and specialties.



Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.



We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.



We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.



Our services are provided in:



Four main hospitals:

Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest

Five community hospitals:

Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire

Two integrated care centres:

Aberaeron and Cardigan in Ceredigion, and several other community settings

47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers

Numerous mental health and learning disabilities services
