Job Purpose

Assist with the delivery of community health initiatives and programmes, including working with individuals and groups to encourage active living and healthy lifestyles. Assist with the promotion, delivery and integration of the key objectives in relation to those identified within the Trust's Service Plan and the Community Plan. Working within a performance management and continuous improvement framework, the post holder will strive to exceed the expectations of the Trust's Strategic Vision and Delivery Plan.
