Job Description

We are looking for enthusiastic pharmacists to join our a friendly, progressive clinical team on bank, serving with Ealing Community Partners at West London NHS Trust. Different shift types will be available including weekdays and weekends.
The successful candidate will be a highly motivated individual with good communication skills and a keen interest in mental health who derives satisfaction from providing an outstanding clinical pharmacy service to the service users and from working with colleagues to continue to develop the service. The applicant should be able to work well both in a team and autonomously.
Main duties of the job
o To support the Lead Pharmacists, integrated care services in the provision of the service.o Responsible for managing and delivering comprehensive medicines management advice and information for clinicians, patients and carers.o Responsible for ensuring the safe, clinically effective and cost efficient use of medicineso To undertake medicine audits on behalf of the pharmacy team.o Provide appropriate training and education for nursing and medical staff with regard to the use of medicines used in psychiatry and other therapeutic areaso Collate activity and outcome data to enhance the evidence baseo Responsible for ensuring a comprehensive flexible pharmacy service is provided to clinical teams. This service should be consistent with best practice and in accordance with national guidelines, in order to support and enhance the holistic care of patients.

• Registered as a pharmacist with the General Pharmaceutical Council
• Post-registration hospital pharmacy experience or equivalent.

Desirable

• Member of RPS
• Non-Medical Prescriber
• Member of UKCPA or other recognised groups/networks for integrated care

• Experience in providing a medicines management service or equivalent
• Knowledge and understanding of the implications of pharmacy national guidance (e.g. NICE, NHS England, MHRA)

Desirable

• Experience in integrated care (or equivalent) environment undertaking clinical medication reviews in older people
• Delivering direct pharmaceutical care to patients with long term condition
• Experience of audit and/or research and development
• Experience working within multidisciplinary teams
• Knowledge of integrated care pharmacy acquired through training and experience
