Job Description

HospiceCare North Northumberland is a local independent palliative charity serving a population of around 63,000 people. We provide high-quality end-of-life-care in patients' homes to the communities of North Northumberland. We also provide emotional and practical care and support to adults and their families, who are living with a life-limiting illness requiring palliative input, through our hospice at home, bereavement support, and dementia support services. Our care is delivered free at the point of need.

We are looking to expand our dedicated Bank Hospice at Home team, so if you are able to work flexibly, commit to a minimum of 20 hours' availability per month and are passionate about person-centred end-of-life care, we would love to hear from you. The post holder will have a key role in providing support to patients and families in their own homes who have palliative and end-of-life care needs. They will also contribute to planning, assessing, implementing and evaluating patient care in partnership with Primary Health Care Teams and Social Services

Main Responsibilities:

Clinical

Assessing, planning, implementing and evaluating care provision within your scope of practice Monitor and evaluate end of life care plans to measure progress and ensure the effectiveness of any interventions Maintaining concise and accurate records of care delivery within care plans and Caring for the Dying Patient documentation Maintain a duty of care to patients, carers and colleagues through the exercise of personal and professional accountability Assess pain and other symptoms using assessment tools, pain history, appropriate physical examination and relevant investigations Provide nursing expertise to patients and contribute to community MDT patient review meetings

Communication/Documentation

Work with patients, their families and friends in a sensitive and flexible manner, demonstrating an awareness of the impact of death, dying and bereavement and recognising that patient priorities and ability to communicate may vary over time Manage unpredictable and/or volatile situations effectively Facilitate communication with clients for whom English is not the first language and those who have communication/language difficulties Understand the range of assessment tools and ways of gathering information and contribute to patient activity data Undertake/contribute to multi-disciplinary holistic assessment and information sharing Demonstrate an awareness and understanding of the legal status and implications of the Advance Care Planning process in accordance with the provisions of the Mental Capacity Act Show an understanding of Informed Consent and demonstrate the ability to give sufficient information in an appropriate manner Work sensitively with families and carers to support them as the patient decides upon their preferences and wishes during the Advance Care Planning process

Leadership, Supervision and Appraisal

Work without direct supervision to deliver care to patients, seeking advice from specialist nurse or members of the Primary Health Care Team when appropriate Work in partnership with Community and district nursing teams, Macmillan nurses, GPs and other health professionals who refer into our services Take responsibility for personal learning and continuing professional development and contributing to the learning of others Promote evidence-based practice through continued professional development and the sharing of best practice within the team and with others Appraise and supervise interventions performed by other members of the nursing team in accordance with Hospice policies Attend Supervision sessions and appraisal as required by the Hospice Promote health and safety, undertaking risk assessments as necessary

Clinical Governance and Quality Standards

Assist in the development of clinical governance and quality frameworks through patient focused benchmarking, clinical audit and critical incident reviews demonstrating learning from multiple sources Implement and adhere to hospice policies In the context of Palliative and End of Life Care have an understanding and knowledge of:
a) Your own professional role/boundaries

b) Legal and ethical issues

c) Professional codes of conduct

d) The impact of your own beliefs on practice

e) Approaches to risk assessment

f) Approaches to and theories of change, loss and bereavement

g) Social models of care and person-centred approaches

h) Relevant NICE Guidance related to Palliative care and End of Life Care

DESIRABLE

Short course in life limiting illness Experience of working in the community Experience of delivering care at end of life Confidence in challenging perceived wisdom and contributes to ongoing service development

Salary: Basic salary 17.12 per hour plus enhanced payments for weekend, Bank Holiday and night work.
