Job Description

JOB PURPOSE

The post-holder will act as the Community Consultant in Palliative Medicine at Pembridge Hospice, have clinical responsibility for referred patients and will be a member of the Pembridge Senior Management Team who lead the service development of the Community Specialist Palliative Care Team, Day Therapy Unit and Medical Out-Patient services.

Our patients are at the heart of all that we do and we work closely with GPs and health and social care partners for a seamless care pathway.



Pembridge Hospice is an NHS funded unit that is part of CLCH and sits within the inner division. There is currently a day hospice for up to 16 patients 4 days per week providing a range of services. There is a large community palliative care team (over 150 patients) covering part of Kensington and Chelsea, South Brent, Hammersmith and Fulham and parts of Westminster boroughs. The CNS team provide a 7 day working service and works collaboratively with other community services.



Provide robust specialist palliative consultant care across care settings, providing diagnostic and treatment support to colleagues as well as advice and support to patients and families.



Provide clinical support to specialist palliative care nurses, other clinical nurse specialists, allied healthcare professionals and other team members, leading change in practice related to supportive and palliative care across the Trust.



Give advice and support to GP's and other community services on patients who are known to and not known to the service via phone.



Lead the multidisciplinary teams delivering specialist palliative care service to community patients, day therapy unit and outpatient attendees.



Be responsible for decisions and patients discussed at the weekly community MDT.



Be responsible for reviewing patients at home and in outpatients as required by the MDT and liaising with GP and others.



Conduct regular caseload reviews of the community CNS's.



Lead on communication and development of seamless service provision with other specialist palliative care providers, GP and healthcare colleagues.



Ensure active engagement and support of End of Life care in the community.



Be responsible for contributing to the strategic development of the units, with regard to locality, End of Life initiatives, and departmental strategy.



Ensure comprehensive handover of clinical information to consultants covering out of hours, ensuring seamless continuity of care.
