Job Title: Palliative Care Social Worker
Location: Calderdale
Pay Rate: Up to 35 per hour
Overview:
Calderdale Council is seeking an experienced Palliative Care Social Worker to join their specialist Palliative Care Social Work Team. This service plays a crucial role in supporting adults (18+) and their families/carers across Calderdale and Kirklees (where health funding is linked to a Calderdale GP).
The team works closely with Health partners to ensure individuals with an end-of-life prognosis receive coordinated, compassionate, and legally compliant support.
Key Responsibilities:
Carry out Care Act Assessments for individuals meeting national eligibility criteria due to an end-of-life prognosis
Support hospital discharge planning where NHS Fast Track funding has been agreed
Work collaboratively with hospital discharge teams and the Integrated Care Board
Complete risk assessments, CHC checklists, DST meetings, and safeguarding enquiries under Section 42 Care Act
Provide ongoing support to individuals and families in the community and local hospice with a prognosis of less than 12 months
Essential Requirements:
Qualified Social Worker (SWE registered)
Experience in palliative care, hospital discharge, CHC, or similar adult services
Strong assessment, safeguarding and MDT working skills
Ability to manage complex and sensitive cases with empathy and professionalism
