Modality Partnership Haydock Division has a part-time vacancy for a Call and Recall Care Coordinator for 20 hours per week. The postholder will be a high performing employee, working across Modality Haydock Division who will proactively support patient care and divisional clinical targets, to help carry out administrative duties principally patient call and recall, including data collection, monitoring, tracking and reporting. Main duties of the job
The role is to help coordinate and recall patients for their chronic disease management, and to ensure that appointments are booked with the appropriate clinician so that the correct care can be delivered to the patients. Working with the management and clinical teams to ensure efficient use of clinical time through active communications management of patients to help reduce DNAs.
The role is not a regular 9am-5pm job; the job requires innovation, flexibility, and commitment and the postholder will be required to work resourcefully as part of the team to ensure tasks are completed.

The job role/duties include but is not limited to:
Help generate call/recall reports highlighting patients due for appointments in a clear and organised way.
Contact patients to book for initial appointments, sending out a maximum of two recall invitations
Remind patients for booked appointments for their chronic disease management
Support with the follow-up of appointments patient DNA
Help coordinate and recall patients for their annual reviews (e.g., Diabetes, Vascular, Hypertension, Respiratory)
Liaise with internal and external agencies / MDT re. patient care
Arrange timely (holistic) appointments for patients and ensure disease specific questionnaires have been provided to patients and submitted ahead of an appointment being booked inc. sending of URL to patient to book an appointment directly
Ensure appropriate appointments are booked with the most appropriate clinician i.e., HCA / Nurse / AHPs / GP
Escalate any issues in a timely manner to the practice / divisional lead
Identifying patients who through disability or illness require alternative means of communication (eg large font, or phone calls rather than standard letters or SMS).
Identify carers and offer them information on the support available through the Division
Notify the patients GP if there are concerns regarding vulnerable adults or children
Participate in Practice / divisional meetings as required.
Work with the management team and clinical team to ensure efficient use of clinical time through active communications management of patients and reducing DNAs.
Monitor and report on key performance indicators for the call and recall systems, working closely with the National Business Intelligence Team
Enter accurate patient information on to the computer as required.
Undertake any other additional duties appropriate to the post as requested by the Patient Services Manager or other senior members of the team
