Cardiology Specialist Nurse
Job Description
To assess, plan, implement and evaluate a programme of Cardiac Rehabilitation as per NICE Guidelines and in line with BACPR standards to cardiac patients in primary and secondary care across the Harrogate and Rural District. To work independently and in conjunction with other members of the team to deliver this high-quality service to patients following cardiac surgery, ACS or PCI through face-to-face consultations and by telephone both in the hospital and in the community. To provide education to inpatients with heart failure in Harrogate District Hospital and onward referral to the community heart failure nurse team.
