About the Role

Were seeking a caring and organised Care Administrator to support our Primary Care Network (PCN) team. Youll play a key role in coordinating care for frail and elderly patients, ensuring they receive joined-up, compassionate support.


Working partly from home and partly from our Gainsborough site, youll help keep services running smoothly by managing administrative tasks, supporting care pathways, and being a point of contact for colleagues, partner organisations and patients.

What Youll Be Doing

Supporting our Frailty Team with day-to-day coordination and administration Maintaining accurate patient records and helping with reports Liaising with colleagues, local partners and patients to ensure continuity of care Supporting induction schedules and training coordination for new team members

What Were Looking For

Experience in health or social care administration Great communication and organisational skills Confidence using Microsoft Office and NHS systems A full driving licence and willingness to travel between PCN practices (mileage reimbursed)

About Us

Trent Care Network (TCN) brings together five GP surgeries across Saxilby, Gainsborough and Hibaldstow, caring for a diverse population of around 40,000 patients. Were an active, collaborative PCN working closely with NHS trusts, the local council and the voluntary sector.
