Job Summary

We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will provide essential support to individuals in their own homes, ensuring their well-being and assisting them in achieving their personal goals. This role requires strong communication skills, a good command of English, and the ability to work collaboratively with both clients and other professionals. It is an essential requirement that you are able to drive and have access to a car.

Duties

Assist clients with daily living activities, including personal care, meal preparation, and medication management. Follow care plans tailored to individual needs, ensuring that all aspects of the client's health and well-being are addressed. Maintain accurate records of client progress and any changes in their condition. Foster a supportive environment by encouraging independence while providing necessary assistance. Communicate effectively with clients, families, and multidisciplinary teams to ensure continuity of care. Utilise IT systems for documentation and reporting purposes, ensuring compliance with organisational policies.

Experience

Previous experience in a care home or home care setting is preferred but not essential. A valid driving licence is required. As is access to a car. Strong communication skills are essential for effective interaction with clients and colleagues. The ability to problem solve and use own initiative is essential. Basic IT skills are required for maintaining records and using relevant software applications.
