Job Overview





We are seeking a dedicated and compassionate Care Assessor to join our domiciliary care team. The role is focused on assessing the needs of service users, developing person-centred care plans, and ensuring care delivery meets individual needs and regulatory standards. The successful candidate will play a key role in safeguarding quality, dignity, and compliance across our care services.

Key Responsibilities




Conduct initial and ongoing care assessments for new and existing service users in their homes. Develop, review, and update person-centred care plans that reflect individual needs, preferences, and risk assessments. Carry out regular review visits to monitor the effectiveness and suitability of care arrangements. Identify changes in service users' needs and ensure care plans are amended accordingly. Work collaboratively with care staff, families, and relevant healthcare professionals to support holistic care delivery. Ensure all assessments and care plans are completed in line with CQC regulations, company policies, and best practice. Accurately maintain assessment records, care plans, and reviews on the company's care management system. Support safeguarding by identifying risks and escalating concerns appropriately. Provide clear guidance to care staff regarding care plans and expected standards of care. Participate in audits and quality assurance activities related to care planning and service delivery. Participate in rotated out-of-hours on-call duties.



Skills and Competencies




Strong assessment and observation skills with a person-centred approach. Excellent communication and interpersonal skills. Ability to work independently and manage a caseload effectively. Good understanding of safeguarding principles and risk management. Confident using care management systems and digital record-keeping tools. Strong knowledge of regulatory and quality standards within domiciliary care.



Qualifications and Experience




Previous experience in a care assessment or senior care role within domiciliary care (essential). NVQ/QCF Level 3 in Health and Social Care (or equivalent) or willingness to work towards it. Sound knowledge of CQC standards and Care Act principles. Full UK driving licence (essential). Access to a vehicle (desirable but not essential).
