We are seeking compassionate and dedicated Care Assistants to provide exceptional day and night care to our residents. This vital role involves supporting individuals with diverse healthcare needs, including those requiring specialized memory care, hospice support, or acute medical attention. The ideal candidate will possess a strong background in various healthcare settings, including long-term care, hospitals, assisted living, and community care environments. Experience with EMR/EHR systems, medication administration, and patient documentation is highly valued. Our Care Assistants play a crucial role in enhancing the quality of life for our residents through attentive, respectful, and personalized care.
Responsibilities
Provide direct patient care across various settings such as hospitals, nursing homes, assisted living facilities, and home care environments.
Assist with activities of daily living including bathing, dressing, mobility support (Hoyer lift), and feeding.
Monitor vital signs and observe changes in patient condition; report findings to nursing staff promptly.
Administer medications following prescribed protocols and document accurately using EMR/EHR systems like Cerner or eClinicalWorks.
Support patients with specialized needs such as dementia, Alzheimer's disease, memory care, behavioral health issues, or post-acute recovery.
Maintain infection control standards through aseptic techniques and proper use of personal protective equipment (PPE).
Document medical records meticulously while ensuring compliance with HIPAA regulations.
Assist with medical procedures such as catheter care or phlebotomy when trained and authorized.
Collaborate with multidisciplinary teams including physicians, nurses, therapists, and social workers to deliver holistic patient-centered care.
Provide emotional support and companionship to patients of all ages, including pediatrics and geriatrics.
Support discharge planning and coordinate home & community care services as needed.
Requirements
Proven experience in healthcare environments such as ICU, emergency medicine, hospital medicine, behavioral health, or post-acute care settings.
Knowledge of medical terminology, anatomy, physiology, and patient safety protocols.
Certification in Basic Life Support (BLS) required; additional certifications such as Phlebotomy or DME are advantageous.
Experience working with vulnerable populations including those with disabilities or developmental delays is preferred.
Familiarity with EMR/EHR systems like Cerner or eClinicalWorks is highly desirable.
Ability to work effectively in fast-paced environments such as Level I Trauma Centers or urgent care clinics.
Strong interpersonal skills for working with diverse patient populations including pediatrics and seniors.
Flexibility to work day shifts or overnight shifts; commitment to providing compassionate patient-centered care around the clock.
Must pass background checks and comply with all health regulations including infection control standards.
