We are now seeking a motivated and well?organised Care Co?ordinator (Non?Clinical) to join our team, based at Lovemead Group Practice, Roundstone Surgery.
About the Role
This role will focus on supporting our local nursing homes and learning disability (LD) homes, ensuring effective communication, coordination, and continuity of care for residents.
You will be responsible for:
Liaising with nursing homes and LD homes to arrange and support routine visits and reviews
Organising clinic and ward round schedules for visiting GPs
Coordinating learning disability health checks and ensuring timely completion of assessments
Supporting vaccination programme planning and scheduling for care home and LD home residents
Maintaining accurate records and ensuring information is shared appropriately across the multidisciplinary team
Providing general administrative support to the PCN care home and LD services
Acting as Carer's Lead, supporting carers within the PCN by ensuring they are identified, signposted to appropriate services, and included in care planning
About You
We are looking for someone who is:
Highly organised with excellent attention to detail
Able to work independently as well as part of a wider multidisciplinary team
A confident communicator, able to liaise professionally with care home staff, clinicians, carers, and patients' families
Essential skills and experience:
Proven administrative experience in a busy environment
Proficient in Microsoft Office (Word, Excel, Outlook)
Excellent communication and organisational skills
Desirable:
Previous experience in primary care or health and social care settings
