The Role:
The successful candidate will play a vital role in ensuring that clients receive the highest standard of care and support. This position requires strong organisational skills and the ability to work collaboratively with healthcare professionals, clients, and their families.
The Care Coordinator will be responsible for managing care plans, coordinating services, and facilitating communication among all parties involved in the client's care.
Care Co-Ordinator responsibilities include but are not limited to:
Develop and maintain individualised care plans tailored to each client's needs.
Coordinate services and resources for clients, ensuring timely access to care.
Communicate effectively with clients, families, and healthcare providers to ensure continuity of care.
Monitor client progress and adjust care plans as necessary.
Maintain accurate records of client interactions, services provided, and any changes in care needs.
Assist clients in navigating healthcare systems and accessing community resources.
Provide support and guidance to clients and their families regarding care options and decisions.
Participate in team meetings to discuss client cases and share insights for improved service delivery.
The ideal candidate will have:
Previous office experience is essential for this role.
SVQ Level 2 (or equivalent) in Health & Social Care is essential to the role.
Strong organisational skills with the ability to manage multiple tasks simultaneously.
Excellent communication skills, both verbal and written, with a focus on empathy and understanding.
Ability to work collaboratively within a team environment while also demonstrating initiative.
Proficiency in using computer systems for record keeping and communication purposes.
