Job summary:
As a Care Coordinator, you will play a vital role in ensuring that our care services are delivered efficiently and effectively. You will have the responsibility of coordinating care staff rotas, liaising with clients and clients family members, and ensuring that high standards of care are consistently maintained.
Requirements:
o? ?To plan, coordinate, and manage care worker's schedules and rotas.
o? ?Handle phone traffic throughout the day.
o? ?Handle new client's referrals and assist with care assessments.
o? ?Participate in the on-call rotas as required.
o? ?Maintain accurate records and documents in line with CQC requirements.
o? ?Monitor and review care delivery, ensuring client satisfaction and compliance.
o? ?Good understanding of CQC regulations and standards.
o? ?Liaise with social services, hospital teams, district nurses etc.
o? ?Have the ability to work well under pressure, and adapt to changing priorities.
Job Types: Part-time, Permanent, Zero hours contract
Pay: 20,838.50-27,147.99 per year
Expected hours: 24 per week
