About the Role - Care Coordinator
We're looking for an experienced, proactive, and organised
Care Coordinator
to support our growing team. Reporting directly to the Branch Manager, you'll play a key role in ensuring our clients receive the care they need
on time, every time
, while also supporting and supervising our frontline care workers.

Key Responsibilities
Create and manage staff rotas
, ensuring carers have appropriate travel time and breaks
Match carers with clients
, considering skills, preferences and location
Handle
new client referrals
from private clients and social services, assisting with assessments and care planning
Liaise with families, clients and healthcare professionals
, providing exceptional customer service
Carry out
risk assessments
,
customer reviews
, and
support worker supervisions
Ensure
client paperwork and care documentation
is accurate and up to date
Support with
weekly KPIs
,
logbook drops
, and
team meetings
Organise and participate in staff supervisions, appraisals, and training
Participate in the
on-call rota
, including alternate weekends (and support other branches when needed)
What We're Looking ForSkills & Experience
Previous experience as a
Care Coordinator
or in a similar
scheduling/rostering
role
Excellent
organisational and administration
skills
Confident using
IT systems
(Excel, Word, care management software)
Experience working in
domiciliary/home care
is
strongly preferred
NVQ Level 2 or above
in Health & Social Care (or working towards)
Personal Attributes
Friendly, professional and confident
Excellent communication skills - verbal and written
A genuine "

A caring nature and passion for improving lives
Able to work flexibly and independently in a busy environment
Team player
with a hands-on approach
Please Note:
This is a
field and office-based role
- so you must be a
driver with access to your own vehicle
You must be willing to work
alternate weekends on-call
and occasionally support neighbouring branches
