We are looking for a passionate and adaptable Care Coordinator to join our expanding team in Barnstaple, North Devon. Role Overview
As a Care Coordinator, you will play a vital role in coordinating and onboarding children and families to our service and the delivery of the care after. This is a full-time position (37.5 hours per week), with the flexibility to work remotely and in-office as we meet in person weekly. We're looking for someone who can embrace the dynamic nature of our growing company--someone who can navigate changes with ease, work effectively with a wide range of people, and bring empathy and compassion to everything they do.
You will work closely with families, healthcare providers, and multidisciplinary teams to ensure children's needs are met, making a significant impact on their overall care and development. Key Responsibilities
Coordination and Case Management
: Manage the intake, assessment, and referral process for children and families. Ensure care plans are implemented, and progress is tracked.
Multidisciplinary Collaboration
: Work closely with a wide variety of professionals to ensure that the service is tailored to each child's individual needs. Act as a liaison between families, external agencies, and our internal teams.
Family Support
: Provide families with Held guidance, advice, and practical support to help them navigate healthcare and education systems, including SEND (Special Educational Needs and Disabilities) services, care services, and access to resources.
Documentation and Reporting
: Maintain up-to-date records in line with legal and professional standards. Ensure all data is entered accurately in systems such as Microsoft 365, SharePoint, Office, and Excel. This would include also include billing, invoicing and reminders.
Adaptability
: Be open to change and willing to evolve with the role as the company grows. You will be expected to adapt to new processes, technologies, and ways of working as we continue to develop as a team.
Engage with Clients and Public
: Be the first point of contact for families, providing empathy and understanding when communicating sensitive information. Manage the main email inbox and prescription inbox.
Training and Development
: Take part in ongoing training to improve your skills and understanding of mental health conditions, including ADHD, ASD, trauma, and attachment disorders.
Confidentiality
: Ability to maintain confidentiality and abide by GDPR, information governance and cyber security policies.

Who We're Looking For -
Qualifications & Experience
:
Experience in healthcare, social services, or a related field in care coordination.
It would be desirable to have had experience working with children and families, particularly those with neurodevelopmental conditions such as ADHD, ASD, or mental health challenges, and familiarity with SEND and care services.
A good understanding of trauma, attachment disorders, and how mental health issues impact children and families.
Minimum of GCSE or equivalent in English and Maths.
Desirable NVQ level 3 in Administration and/or Care Coordination.
Skills
:
Proficient in Microsoft 365 (including Word, Excel, SharePoint, and Office), there will be a test as part of the interview process.
Excellent communication skills, with the ability to engage effectively with the general public and a diverse range of professionals.
Strong organisational skills, with the ability to manage multiple cases and competing priorities.
Experience working in a healthcare or care coordination setting is desirable.
