Ward: Croydon Mental Health Learning Disability Service

Pay Rates: 21.96 per hour

Shift Patterns: Mon-Fri 9am-5pm (part-time accepted)

What you can add:

Act as Care Coordinator under CPA, providing holistic support, risk management, and effective liaison with service users, families, carers, GPs, and other services. Build psychologically safe environments that promote inclusion, collaboration, and support for diverse service users, families, carers, and staff. Develop and maintain person-centred care plans, risk assessments, and specialist nursing interventions tailored to individuals with learning disabilities, autism, and complex mental health needs. Attend and contribute to weekly team meetings and case formulation discussions helping to provide specialist support/advice to other professionals and support effective multidisciplinary care for clients. Deliver bespoke training to a range of stakeholder organisations and providers in Croydon of the service user group and pathway.
What you'll need:

Registered with either RMN, RNLD or HCPC Degree-level education Experience of working within a community mental health setting is essential Post-qualification experience working with adults with learning disabilities and mental health needs, or strong evidence of transferable skills Strong clinical knowledge in supporting clients with complex needs and challenging behaviours Understanding of evidence-based practices (e.g. PBS, ABA, NICE guidelines) Knowledge of legislation including MHA, CPA, Human Rights Act, Child Protection policies Skilled in risk assessment and care planning Confidence participating in MDT meetings and contributing to clinical discussions
Desirable:

Experience working in partnership with families and carers Car owner (preferred but not essential) EPJS experience
