The Role



We are seeking a

compassionate, highly organised and motivated Care Coordinator

to join our growing team in

Beverley

. You will play a vital role in the effective management and delivery of community care services, ensuring our clients receive outstanding, person-centred support tailored to their individual needs.

This role requires excellent organisational ability, clear communication and a proactive approach to problem-solving.

Key Responsibilities



Client Management



Develop, review and maintain individual care plans Ensure care is delivered in line with clients' needs, preferences and best interests

Scheduling & Coordination



Plan and coordinate staff rotas and care visits Ensure efficient use of resources while maintaining continuity of care

Care Quality & Monitoring



Monitor the quality of care delivered by support staff Conduct spot checks and reviews to ensure compliance with company standards

Liaison & Communication



Act as the main point of contact for clients, families and care staff Maintain clear and effective communication at all times

Recruitment & Training Support



Assist with recruitment, induction and ongoing staff training Provide guidance and support to the care team

Compliance & Regulation



Ensure all services meet

CQC standards

and regulatory requirements Support the Registered Manager with audits and inspections

Problem Solving



Manage and resolve any concerns or issues promptly and professionally

Record Keeping



Maintain accurate records including care plans, visit logs and service reports

On-Call Duties



Participate in an

on-call rota

to ensure operational support

Essential Skills & Experience



? Previous experience in a

Care Coordinator, Care Manager or similar role

? Strong understanding of

person-centred care

? Excellent

organisation and time management

skills ? Confident communicator with the ability to build strong professional relationships ? Working knowledge of

CQC standards and compliance

? Proficient in

care management software and Microsoft Office (Word, Excel)

? Ability to work independently and as part of a team ?

Full UK Driving Licence & access to a vehicle (essential)

?

NVQ Level 3 (or equivalent) in Health & Social Care

? Experience within

domiciliary/home care
