We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that clients receive the highest quality of care and support. This position involves coordinating various aspects of patient care, liaising with healthcare professionals, and providing administrative support within an office environment. The Care Coordinator will be responsible for facilitating communication between clients, families, and healthcare providers to ensure seamless service delivery.
Duties
Coordinate and manage client care plans, ensuring all services are delivered effectively and efficiently.
Serve as the primary point of contact for clients and their families, addressing any concerns or questions regarding their care.
Liaise with healthcare professionals to facilitate appointments, referrals, and follow-up care.
Maintain accurate and up-to-date client records in compliance with relevant regulations and policies.
Assist in the development of care strategies tailored to individual client needs.
Organise office operations, including scheduling meetings, managing correspondence, and maintaining filing systems.
Monitor client progress and report any changes or concerns to the appropriate healthcare team members.
Participate in team meetings to discuss client cases and share insights for improved service delivery.
In addition, you will be required to do health care assistant and senior care assistant duties as needed by the firm.
Skills
Strong organisational skills with the ability to manage multiple tasks simultaneously.
Excellent communication skills, both verbal and written, to effectively interact with clients, families, and healthcare professionals.
Proficiency in office software applications (e.g., Microsoft Office Suite) to maintain records and documentation.
Empathy and compassion for clients, demonstrating a commitment to providing high-quality care.
Ability to work collaboratively within a team environment while also being self-motivated.
Attention to detail to ensure accuracy in documentation and client care plans.
Diving License Required with own car.
