Job Overview


We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that clients receive the highest standard of care and support. This position requires strong organisational skills and the ability to manage multiple tasks efficiently within an office environment. The Care Coordinator will liaise between clients, families, and healthcare professionals to facilitate effective communication and care delivery.

Responsibilities



Enable access to personalised care and support Work closely with Senior Management in completing care needs assessment for clients. Complete care and support plan to ensure appropriate support is in place for the person. Review and update personalised care and support plans at regular intervals. Provide one to one supervision and direct observations to care staff working in the community. Complete relevant documentation for clients to aid their care services, including Medication Administration Records (MARs sheets), fluid charts, body maps, accident forms etc. Take referrals for individuals or proactively identify people who could benefit from support. Have a positive, empathetic and responsive conversation with the person and their family and carer(s) about their needs. Work towards increasing clients' understanding of how to manage and develop health and wellbeing through offering advice and guidance. Develop an in-depth knowledge of the local health and care infrastructure and know how and when to enable people to access support and services that are right for them. Use tools to measure clients' levels of knowledge, skills and confidence in managing their health and to tailor support to them accordingly. Support clients to develop and implement personalised care and support plans. Complete onsite spot checks for support workers to promote performance in the care services. Coordinate and integrate care
Supervising and monitoring care and home appointments for clients.

Regularly liaise with the range of multidisciplinary professionals and colleagues involved in the person's care, facilitating a coordinated approach and ensuring everyone is kept up to date so that any issues or concerns can be appropriately addressed and supported. Actively participate in team meetings. Identify when action or additional support is needed, alerting Senior Management Team highlighting any safety concerns. Keep accurate and up-to-date records of contacts, appropriately using client records systems, adhering to information governance and data protection legislation. Work sensitively with people, their families and carers to capture key information, while tracking of the impact of care coordination on their health and wellbeing. Encourage people, their families and carers to provide feedback and to share their experiences about their care. Record and collate information according to agreed protocols and contribute to evaluation reports required for the monitoring and quality improvement of the service.

OTHER TASKS



To cover care shifts where necessary in case of staff sicknesses, and be able to provide personal and practical care services to clients with a wide range of vulnerabilities, illnesses and needs. To help care for clients who may be in the final stages of life and treat them with absolute dignity and respect. To read and to write clearly in the client's communication logbook, medication forms and financial transaction sheets to ensure an accurate record of activities undertaken and the status of the client at the time of each visit. To record accurately any changes that have occurred and other relevant information. To provide a signature with each entry and be mindful of comments written so as not to unintentionally offend or upset clients or their family/representatives as they are entitled to read the communications log. To be vigilant regarding the client's needs and condition and report to the manager(s) any safeguarding issues which in the Care Coordinator's opinion warrants investigation or urgent attention. To report immediately to management any noticeable changes in health, behaviour or circumstances of the client whilst maintaining the client's right to privacy and confidentiality. To be available to work as and when agreed with Eclectic Care Ltd and give appropriate notice of unavailability. To carry out accurately and in a competent manner, instructions from the manager adhering to the care plans of individual clients. To be pro-active when talking and listening to clients, allowing them to make their own personal choices in their own home. To advise the Registered Manager of any ideas that may improve the level of service delivered to clients. To work only to Eclectic Care Ltd.'s regulations regarding the handling of client's finances, ensuring accurate documentation and receipting of any transactions. To be familiar with Eclectic Care Ltd.'s

. To record and report any accident or incident which may occur, no matter how minor or whether an accident or incident has occurred to a client or care worker. To ensure the Company's

are followed when dealing with any concerns or complaints raised by clients or their carers. To understand and comply with both Company and legislative requirements regarding confidentiality and data protection. To conform to all policies and procedures laid down by Eclectic Care Ltd. relating to carrying out the above duties and in other duties as may be required to ensure the smooth running of the company. To participate in induction and regular in-house training for care staff. To be available for an annual Personal Performance Review which will be recorded in their personnel record.

Qualifications



Previous office experience is essential, demonstrating strong administrative skills. Excellent communication skills, both verbal and written, with a compassionate approach to client interactions. Strong organisational abilities with attention to detail, capable of managing multiple priorities effectively. Proficiency in using office software and electronic health record systems. A background in healthcare or social services is advantageous but not mandatory. Ability to work collaboratively within a team environment while also being self-motivated.
