We are seeking a compassionate, organized, and proactive Care Coordinator to join our healthcare team. The ideal candidate will serve as a vital link between patients, healthcare providers, and support services, ensuring seamless care delivery and an exceptional patient experience.
Key Responsibilities
Serve as the primary point of contact for patients, families, and multidisciplinary teams.
Coordinate appointments, referrals, follow-ups, and transitions of care.
Monitor patient progress and ensure timely communication of care plans.
Advocate for patient needs, addressing barriers to care such as transportation, financial assistance, or language support.
Maintain accurate and confidential patient records using electronic health systems.
Collaborate with clinical staff to optimize care pathways and improve outcomes.
Educate patients and families on treatment plans, medications, and available resources.
Participate in quality improvement initiatives and case reviews.
Qualifications
Degree or certification in healthcare, social work, nursing, or related field preferred.
Previous experience in care coordination, case management, or patient advocacy.
Strong interpersonal and communication skills, with a focus on empathy and professionalism.
Proficiency in healthcare systems and electronic medical records (EMR).
