We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will be responsible for organising and overseeing care plans, liaising with clients and healthcare professionals, and ensuring the delivery of high-quality support services. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring assistance, while working in a supportive and professional environment. Duties
Coordinate and manage personalised care plans tailored to individual needs
Liaise effectively with clients, families, and healthcare providers to ensure seamless communication
Schedule appointments, visits, and services in accordance with care requirements
Maintain accurate records of care activities and client information using organisational systems
Monitor the quality of care delivered and address any issues promptly
Assist with administrative tasks such as documentation, reporting, and data entry
Support clients in accessing community resources and additional support services as needed
Ensure compliance with organisational policies and relevant health and safety regulations
Skills
Prior experience working in an office environment is desirable but not essential; organisational skills are vital
Excellent communication skills, both written and verbal, with a compassionate approach
Strong organisational abilities with attention to detail
Ability to multitask effectively whilst maintaining professionalism under pressure
Proficient in using standard office software programmes (e.g., Word, Excel)
Ability to work independently as well as part of a multidisciplinary team
Empathy and understanding towards clients' needs and circumstances
