Duties
- Coordinate and manage patient care activities
- Schedule appointments and follow-up visits
- Communicate with healthcare providers, patients, and their families
- Assist in the development and implementation of care plans
- Monitor patient progress and update records accordingly
- Provide education and support to patients and their families
- Maintain accurate and up-to-date documentation
- Collaborate with interdisciplinary teams to ensure quality care delivery
- Maintain accurate and up-to-date documentation
- Monitor calls and provide on-call cover to
- Provide hands on care if required in staff absence
Requirements
- High school diploma or equivalent required; associate's or bachelor's degree in healthcare or related field preferred (QCF level 3 or above)
- Proven experience in a healthcare setting, preferably as a care coordinator or similar role
- Knowledge of medical terminology and healthcare procedures
- Proficient in using office software (e.g., Microsoft Office Suite)
- Excellent organisational and multitasking skills
- Strong communication and interpersonal abilities
- Ability to work independently and as part of a team
- Attention to detail and accuracy in record keeping
