Job Overview
As a
Care Coordinator
, you'll play a vital role in ensuring service users receive the highest quality of care. You'll act as a key link between service users, healthcare professionals, and support services--ensuring care is well-coordinated, responsive, and tailored to individual needs.
Key Responsibilities
Coordinate appointments, follow-ups, and treatments for service users
Serve as the main point of contact, responding to enquiries with empathy and professionalism
Maintain accurate records in line with healthcare regulations
Collaborate with professionals to develop and review individualised care plans
Monitor progress and adjust plans to ensure continuity of care
Support office operations including rostering, payroll, staff supervisions, and resource management
Assist service users in accessing healthcare systems and community resources
Travel within Chester and Cheshire West
Experience & Skills
Previous experience in health or social care is preferred but not essential
Strong desire to support others and make a positive impact
Excellent communication and interpersonal skills
Organised and detail-oriented, with the ability to manage multiple priorities
Proficient in administrative tasks; familiarity with care software is a plus (training provided)
NVQ Level 3 in Health & Social Care (or willingness to work towards it)
Driving is essential
