The Care Coordinator will play a vital role in ensuring that clients receive the highest quality of care by coordinating services, managing schedules, and acting as a liaison between clients, families, and healthcare providers. Responsibilities
Coordinate care services for clients by assessing their needs and preferences
Schedule appointments and manage calendars for healthcare providers
Communicate effectively with clients, families, and medical professionals to ensure continuity of care
Maintain accurate records of client interactions and care plans
Assist in the development of care strategies tailored to individual client needs
Monitor client progress and make adjustments to care plans as necessary
Provide administrative support within the office, including filing, data entry, and responding to inquiries
Requirements
Previous office experience is essential for this role
Strong communication skills, both written and verbal
Excellent organisational abilities with attention to detail
Ability to work independently as well as part of a team
Proficiency in using computer systems and software for documentation and scheduling
