Care Coordinator
to support our residents and team in delivering outstanding care every day.
Role Overview:
As Care Coordinator, you will oversee the planning and delivery of personalised care for our residents, ensuring their needs, preferences, and wellbeing are always prioritised. You will play a vital role in supporting both residents and their families while guiding and supervising care staff to maintain the standards Hyperion House is proud of.
Key Responsibilities:
Person-Centred Care Planning:
Create, review, and adapt care plans tailored to each resident's needs and preferences, ensuring dignity, independence, and wellbeing are at the heart of every plan.
Resident Advocacy:
Work closely with residents and their families, acting as a trusted point of contact to ensure care is transparent, collaborative, and responsive.
Staff Supervision & Support:
Supervise daily care delivery, providing ongoing support, guidance, and mentorship to the care team. Conduct informal check-ins and formal reviews to maintain and enhance performance standards.
Training & Development:
Assist in the onboarding of new team members, ensuring they are fully briefed on Hyperion's values and standards. Provide training and coaching to reinforce best practices and promote professional growth.
Team Coordination:
Work collaboratively with nurses, carers, therapists, and activity coordinators to ensure a joined-up, holistic approach to resident wellbeing.
Scheduling & Coverage:
Support rota planning to ensure appropriate staffing levels at all times, with continuity and consistency of care.
Quality Monitoring:
Uphold and promote Hyperion's high standards by monitoring care delivery, identifying areas for improvement, and supporting a culture of accountability and excellence.
Record Keeping & Compliance:
Maintain accurate care records in line with regulatory and internal standards, ensuring timely updates and audit readiness (see below)
Audits:
Carry out and review audits in line with KPIs
Community Spirit:
Help foster the homely, inclusive environment that makes Hyperion House feel like family--for residents, relatives, and staff alike.
The successful candidate will be IT literate and have experience of using electronic care planning systems.
Job Types: Full-time, Permanent
Pay: 37,500.00-42,000.00 per year
