Are you a care co-ordinator looking to join a fabulous team? Look no further.





Live in Care is a privately owned and run company that specialises in live-in care and hasn't lost that personal touch. This is a full-time position, based at our modern head office in Taunton.



You will utilise your experience and expertise to support our live-in carers, clients, and family members effectively and professionally, and will be actively involved in planning our live-in carer placements to ensure that our clients receive the quality care they deserve. You will need a logistical and organised mindset.



We require an exceptional communicator, as the successful candidate will be communicating daily via telephone and email with carers, clients, and family members. You must be computer literate and confident to use the electronic systems we have in place (training will be provided). Accuracy, efficient organisational skills, and time management are key requirements of this role. You will need a minimum of 2 years' experience in care and be a confident, professional person. This may be a suitable role for someone looking to transition from a care-based role to a primarily office-based one.



We offer an excellent remuneration package, including 28 days of annual leave (with bank holidays), yearly increments, a company pension scheme, an employee assistance programme, and team events.



Working hours are Monday to Friday, 9:00 a.m. to 5:00 p.m., with a primarily office-based schedule. However, you will need to be a driver to support client visits, and a company car is available for this purpose.



You will be expected to participate in an out-of-hours on-call service (currently 1 weekend in 6).


As a Care Co-ordinator, you will be responsible for:




Ensuring our clients receive the highest standards of care and support by completing regular reviews in person to ensure care plans reflect the support required and individual client preferences accurately. Updating electronic care plans and MAR charts. Completing carer support visits and direct observations. Making referrals to multi-disciplinary teams to provide integrated care. Auditing evidence of care provided. Providing additional support and guidance to our live-in care professionals. Ensuring the highest level of customer service and satisfaction - answering calls, managing outcomes, completing quality assurance checks. Supporting our Care Team Lead with day-to-day requirements.

Essential experience within the care sector with knowledge of regulatory requirements and best practice.



Preferred minimum level 3 Diploma in Adult Social Care or willing to work towards achieving this.

Salary: 26,000 - 28,000
