Overview


We are looking for an organised and compassionate

Care Coordinator

to support the day-to-day coordination of care across our supported living and domiciliary services. This role plays a key part in ensuring safe, person-centred care delivery while supporting operational teams and service users.

You will work closely with managers, staff, service users, and external professionals to ensure care is delivered in line with individual care plans, regulatory standards, and organisational values.

Responsibilities



Organise and schedule appointments, treatments, and care plans for clients in accordance with their individual needs. Liaise effectively with healthcare professionals, service providers, and families to coordinate care services. Maintain accurate and up-to-date records of client interactions, care plans, and progress notes using office systems. Conduct daily check-ins and virtual meetings with staff. Provide information and support to clients and their families regarding available services and resources. Manage rotas, staffing levels, and shift cover using e-rostering systems. Escalate incidents, complaints, and safeguarding concerns appropriately. Assist in the preparation of reports and documentation required for audits or quality assurance processes.

Qualifications



Experience in health & social care coordination or a similar role. Strong organisational skills with the ability to manage multiple priorities efficiently. Excellent communication skills, both written and verbal, with a compassionate approach towards clients. Ability to work independently as well as part of a multidisciplinary team. Proficiency in using office software such as (e-rostering, HR, care records,). Knowledge of safeguarding, care standards, and person-centred care.
