We are seeking a compassionate, organized, and proactive Care Coordinator to join our team. This pivotal role ensures that individuals receive the right care at the right time by coordinating services, advocating for clients, and supporting multidisciplinary teams. The ideal candidate will have strong interpersonal skills, a keen eye for detail, previous senior care work experience and a genuine passion for improving lives.
Key Responsibilities
Serve as the primary point of contact for clients, families, and care providers.
Develop, implement, and monitor personalized care plans in collaboration with healthcare professionals.
Coordinate appointments, referrals, and follow-ups across services (e.g., medical, social, mental health).
Maintain accurate and up-to-date records in line with data protection and confidentiality policies.
Advocate for clients' needs and ensure their voices are heard in care planning.
Monitor progress and adjust care plans as needed to reflect changing circumstances.
Facilitate communication between internal teams, external agencies, and service users.
Support discharge planning and transitions between care settings.
Spot checks

Skills & Qualifications
Experience in health, social care, or case management (minimum 1-2 years preferred).
Relevant qualification in health and social care, nursing, or related field.
Strong organizational and time-management skills.
Excellent communication and interpersonal abilities.
Empathy, patience, and a client-centred approach.
Proficiency in digital record systems and Microsoft Office.
Knowledge of safeguarding, confidentiality, and care standards.
Desirable Attributes
Experience working with vulnerable populations (e.g., elderly, disabled, mental health).
Familiarity with local health and social care systems.
Ability to work independently and as part of a multidisciplinary team.
