Currently, we are actively seeking an experienced full-time Care Coordinator to join our client organisation Chosen Care Group.

In the pivotal role of a Care Coordinator, you become an integral link between our care workers, clients, Next of Kin (NOK), and professional counterparts. The successful Care Coordinator will have:



Previous Care Coordination experience Excellent customer service skills

Used planning software such as People Planner, CM2000, etc

The ability to work well under pressure, good time management skills and use your initiative Have good influencing skills - proven ability to convince others and gain their respect. Excellent communication skills - reasonable telephone manner and the ability to build relationships, listen and explain things clearly at all levels. A willingness to participate in an on-call system for out-of-office hours on a rota basis Have an excellent working knowledge of Microsoft Office packages

Main duties -



Manage the day-to-day operations of the care service, ensuring that the highest standards of care are provided to all patients. Develop and maintain staff rotas to ensure adequate coverage for Patient care, activities, and emergencies while optimising staff scheduling. Monitor compliance with regulatory standards, policies, and procedures to ensure a safe and secure patient environment. Collaborate with patients and their families to develop personalised care plans that address their unique needs, preferences, and goals. Train staff and ensure that all staff are adequately trained and supervised to provide the best possible care for each patient. Oversee the safe and accurate administration of medications, ensuring adherence to prescribed schedules and proper documentation. Facilitate access to external support services, healthcare professionals, and specialists as needed to enhance patient's overall quality of life.
