About the Role:
We are looking for a compassionate, highly organised, and proactive Care Coordinator to join our team. This is a key role within our care agency, responsible for the scheduling and coordination of care visits to ensure clients receive high-quality, timely, and person-centred support. You will play an essential part in managing day-to-day operations, ensuring that clients' needs are met, care staff are efficiently allocated, and that communication between clients, carers, and management is seamless.
You will be responsible for ensuring that all care visits are scheduled appropriately, maintaining accurate records, and managing changes in care plans or staffing requirements. Key Responsibilities:
Care Scheduling & Allocation: Schedule and allocate care visits to the right staff based on client needs, preferences, and staff availability, ensuring timely and efficient care delivery.
Care Management System: Maintain accurate records of client care plans, schedules, and visit outcomes in the care management system.
Communication & Liaison: Serve as the key point of contact for clients, families, and care workers. Handle any queries, concerns, or changes in care delivery in a professional and timely manner.
Staff Support & Development: Assist with the recruitment, onboarding, and training of new care workers. Ensure staff are well-supported and equipped to deliver high-quality care.
Compliance & Standards: Ensure all care practices comply with CQC standards, local regulatory guidelines, and internal company policies.
On-Call Support: Participate in the on-call rota (if applicable) to address urgent issues and make adjustments to care schedules as needed.
Quality Assurance: Monitor the quality of care being delivered and take appropriate action where necessary to ensure the highest standards of care are maintained.
Requirements:
Must have Level 5 Leadership and Management (or equivalent) qualification.
Must have NVQ Level 5 in Health and Social Care (or equivalent).
Previous experience in a Care Coordinator or similar scheduling role in the health and social care sector.
Strong communication and interpersonal skills to engage effectively with clients, families, and care workers.
Excellent organisational and time management skills, with the ability to manage multiple tasks and prioritise effectively.
Good IT skills and experience using rostering software and care management systems.
Knowledge of CQC standards, care planning, and regulatory requirements in the care sector is desirable.
